Our practice hasn’t got a lead doctor so it could be anyone as is part of a group of practices so you couldn’t put on the form who your gp is. The next thing is if they run you out of time and your licence expires you have to start with a new licence application which is a whole new can of worms...

It’s not the doctor you are dealing with, it’s the practice manager. Since our rural practice was taken over by a town centre one and then shut we don’t have an allocated doctor just whoever is available. I don’t suppose a doctor has anything to do with this.

You download the doctors form but the rest you fill out on line, they say it takes 20 minutes but you can’t save it as you fill it in so if you can’t complete it in one go you start again.

There is no legal obligation on them at the moment to do this. I looked at moving doctors as another surgery will do these but due to COVID no new patients are being taken on

Sussex police won’t do that , without the form being filled out and stamped by a gp they won’t even process your renewal

They have to give you a copy but in my case first you have to request a form then send it to them , they then have 28 days to do this. Problem is I am still waiting for the form.
