My customers who are having a similar problem have been going back to NE with a specialist mixture less the deeper rooting species like Cocksfoot, Lucerne and Vetch for example.

Soil temperatures need to be at least 5°C before sowing. Hopefully, that will be early April with us in the Sout West. Sowing earlier in cold conditions can lead to bolting.
Aim for a 7pH and drill to a depth of 2.5-3cm.
Cocksfoot can be very susceptible to Mastigosporium which looks similar to your photo.
This fungi favours wet conditions.
What part of the world do you farm in, I see it down in my part of the world.
One of my suppliers recently received an order for 18 tonnes of seed from one holding and another for sowing 1800 acres which would have been cereals. I had some big orders too!
