It is quite easy to find case summaries. Just go to HSE website and it is all there

Nobody looks them up though because, elf n safety innit.
Something on here that you could view easily and move on might be better.
A detail might stick in your memory and prevent you repeating a mistake.

Shows like Air crash investigation always explain at the end, how the findings of the investigation have been used in the industry to prevent further deaths.

A voluntary "confession" thread would not work in my view, as we never like to admit to our shortcomings, especially where H&S indiscretions are concerned.

But, in general I think it's a great idea.

The whole H&S brand has had a rough life, especially when we have to read about the Police not entering the water to save someone drowning due to H&S policy and procedures. Not allowing kites to be flown on beaches, Banning conker fights, snowball fights, the sack race, the three legged race and so on. Then we wonder why we have a snowflake society.

To a degree, corporate liability has allot to answer for.

Sadly, H&S is UK Agriculture's grey underbelly and these very sad accidents happen all to often.

However, most other Industries also have H&S issues, the only difference being if it's a ISO registered business, all LTI's go on a record, so H&S is managed much better.

I think that one of the biggest issues is that there's so many misconceptions about H&S and HSE. I enjoy being told by the ignorant that such and such is "banned by law" because of "elfin safety", because nine times out of ten it's total nonsense & almost always is one of the myths busted on the HSE website, such as https://www.hse.gov.uk/myth/september.htm

The reality is that silly rules get made up by people who don't know where to look for guidance, plus those who like to don a hi viz and wave a clipboard!
 
Lack of concentration, absent mindedness, and distractions must be three of the biggest killers in all industries; as well as in traffic accidents. Oh, I almost forgot tiredness!

The incident in the OP is, of course, extremely traumatic for the family, friends, and all concerned and as has already been pointed out, open forum is not the place for such details to be exposed at this time.

What, may I ask, do the farming communities do for families in such sad circumstances?
 
We say a few kind words, then bury our heads again !

I suppose that the same used to apply to suicides but many folk are now seeking ways to support people and avoid such events. Several poster in this thread have suggested a locked thread listing major farming accident details where we can all read, learn, and inwardly digest such matters; sounds like a good idea to me but who, I wonder, would collate such a thread?

If you dig a wee bit deeper into cases you will see examples of outcomes and why things went wrong. We need to apply it too our own situations.
eg. a tractor would not start, it had been reported as needing repair but someone jumped it across the solenoid and was run over and killed for sake of a 30 minute repair when it should not have been used.

The aircraft reporting template looks far too detailed.

Agreed but It gives an idea

We really need such summaries for as many incidents as possible to get a picture of the causes and frequency.

when I worked for a utility they shared fatal accident details so we could learn from them, these were shared between all utility companies and we would go through them at team meetings.

as everyone has said makes you think twice somthink like that for agri would be very useful, even listening to chaes account will make me more carful next time I’m loading fert.

I looked at ones on gov website not really format I expected as others say need something simple with outcome lead up etc.

just a thought.
There are many people on here from other industries, such as marine, air, rail etc. Each of these industries have excellent investigation systems and reports.
As mentioned above, it would be an excellent idea, in my opinion, if such a thing existing for agriculture, to collate the reports in a section with no comments allowed - a library to learn from.
If an agriculture one does nto exists, maybe we could collate reports from other industries, as it is often recognised on here how the lessons are transferable across various idurstried?

good idea - i have a pilot’s licence and get sent results of accident investigations , they are a great reminder of the dangers and very helpful / sobering despite often being hard reading

maybe the HSE should mail out similar publications ?

Much the same in the chemical industry and Aviation Security but in Aviation Security a great deal was kept in-house and only shared with other international aviation security bodies.
