I heard about this, the people he worked for operate out of a yard I delivered straw to today. By all accounts he was very experienced operator and they are hot on risk assessments and method statements. However, have you read some of the comments in the link. It's beyond belief, I can't understand the mentally. It should be illegal to write such stuff under the circumstances especially when they haven't a clue about the situation.
