Not sure what the rules are quite, but I am certain what I saw yesterday on the A47 was quite wrong.
A very large load taking up both lanes completely on one side of a dual carriageway with 2 police motorbikes and a car on the blues and twos.
Except they were all in front , does not make a lot of sense when the traffic was piled up behind.

If traffic can’t get past and they’ve slowed traffic in both lanes there’s no longer much point been at the back. Couldn’t tell you what the exact rules are in this situation but who’s going to argue with the rozzers!

I "thought" you had a tail vehicle on dual carriageway, but as you say there wouldn't have been anyone approaching from behind in that situation.

Surely most "escorts" are approachable from behind!
Actually I’m led to believe that depending on the situation “escorts” can be approached from multiple directions sometimes at the same time! Obviously, it will depend on the experience of the “escort” how they deal with these situations and what the outcome is.

ps, I would think in some situations it’s better to use multiple “escorts”
 
If the traffic is already queued behind and the load is sufficiently wide that no one could even attempt a dangerous overtake, then it probably makes more sense for the escorts to go in front. That way they can clear roundabouts and junctions and ensure a clear path ahead. I've seen them doing that before.
