Little square and IMG after it

I keep getting the above "icon-like" thing in posts , where presumably there should be an image posted . Why is this please , and how can I remedy this so that I can see the image ? . I also occasionally get the message "You do not have authority to access this " (or similar ) Why is this please ?

That normally means an image has been loaded incorrectly, but also can happen when a connection is interrupted when loading a page. Is there a page where you always see it, do does it just come and go?

Not really sure , sometimes it's there sometimes not , but I've never checked exactly when or under what circumstances . Doesn't seem to matter which browser I use . sometimes "Chrome" others "Opera" others The Windows browser . I'm fairly sure it isn't just one page though , if I get one , it's just as likely I'll get another .
