Witness testing

Tech question.
Had a witness test done 3 years ago
asked for evidence and all I have been given is an email confirming and an apparent reference code from UKPN
it is 9 figures dash then another 9 figures
is there any central register of such things?

DNO are supposed to have a copy on file as proof. From experience our local lot fill their own form in aswell as the tester filling in his/hers. Who did the test?
