when you push the pipe in waggle the lever around that pushes the pipe out when you take them out .ours did this all the time with certain machines and I think the lever just doesn't go back to the middle when you push the pipe in and its then only half held the pipe so it just blows out when...

i think the speed selector has to be in the middle ie neutral to put the light off and make it start it will have a press switch either under the leaver or at the far end of the cable that will probably want some wd40 on to make it pop back out
