Do you know if it shows any more error codes other than that one. There is one possibility that shouldn't be overlooked with your symptoms. It may be the rear pto torque sensor. If this is failing or isn't close enough to the sensor wheel it can cause some of your problems.

What are the gears it mainly does it in?If there is one clutch pack associated with all the gears there could be a fault there with the pack causing it to loose it's low pressure as that is what the clutch packs work off.
