yes it happens had one some years ago, blade came off when mowing a field near houseing sestate, went thriugh a lounge window , and landed on a sofa , the man of the house was out that evening but was his normal seat for watching telly, they were very nice about it all iirc, , i just got insurance to put in new double glazed unit by a local (not rip off) firm
Bolt on or quick change? Don't think I've ever completely lost a bolt on.

Lost a back window from a stone once.

springy holder/quick change but i think the actuall reasoin was a damaged blade 9by a stone or what ever ) rather than total holder weakness ,long time ago tho cant ttotally remmeber about it.

Those circular saw blades that fit on the end of hedge trimmers frighten me . Imagine if that thing came off!
I built my own circular saw for logs. I tested the emergency stop early on in the development by pulling the string to the stop button on my 35. The shaft stopped as soon as the engine did but the blade didn't. It kept spinning and in the process undid the big nut holding it on.
I knew what had happened but didn't know whether to run, duck or what. Fortunately the blade just spun on the shaft until it stopped but if it had come off the end of the shaft I have no idea what would have gone.
I built a cage around the blade after that, if it happens again it will be inside the cage but even that would be pretty scary. In reality you only need the emergency stop if there is a log jammed so the blade isn't freewheeling anyway.

Surely blade nut should have been left hand thread, ??

I do wonder if these quick change holders are worth the time saved. I know I've lost 3 or 4 blades out of the holders. Could be stones or clump of sod that dislodges them? I think bolt-on would without a doubt stay put even if colliding with something. I don't do masses of mowing so wonder if changing back to bolt-on blades is the best solution? Must admit I too run Krone mowers, front and rear.
 
A local farmer switched from kverneland trailed to krone he said he never managed to loosed blades off the kverneland but the krone plenty that was one thing that stood out for him have two kuhns with quick fit and no blade lost but have hit good big stones from time to time going to near ditches and they remain in place local contractor with a big m say the holders wear on the quick change
When out mowing the other day and a blade came off the mowers ,cleared 10ft hedge a main road another 10ft hedge then crossed a big garden and when in to the house by breaking a double glazed door ,then hit a wood burning stove and then ended up on a kitchen worktop. form the mower to the house would be 100 metres 5 mins before that the lady and kids were watching the mower from that very window. We were very luck

Slightly off topic but similar;
There was an article about timber harvesting and the stupidity of the general public ignoring the warning signs and entering into the harvesting machinery's danger zone.
Having seen ‘chain-shot’ and also lost a mulcher hammer, from a poor forging, I can well understand how a mower blade could travel so far.

Someone my dad went to school with had a couple of fingers chopped off with a mower blade when he was a kid.

He was stood leaning against a fence post, hands on the top of it with his head resting on his hands watching the farmer cutting when a blade hit the top of the post chopping his fingers off. If it hit 6 inches further up, left or right he would of been dead.

We were all told that story every year when the mower came out. It definitely made you think twice about going anywhere near it.

Jeez!! That was very unlucky and dangerous all in one! Normally they skin across the ground and speed gets scrubbed off but sounds like yours snapped and took off?
