There are often break clauses, and other provisions for one or both sides.
A landlord seldom gives property away for longer than 3 years without some mechanism for regaining vacant possession.

You are correct. To bring it to an end on the 5th anniversary, as agreed, 12 months notice needs to be given ahead of that date. Otherwise it rolls on from year to year.
