Should be charged by the hour. Big difference between 2cwt an acre in fields surrounding the yard and say 3cwt an acre 3 miles away half way up a mountain..
So man A does 40 acres hour man B does 5 acres hour same land .that's why it's done per acre so it rewards those who do more

Depends on application rate? Could be 500kg/ha or 125kg/ha. Field size, terrain? Could be a 100 acre flat square field or side of a hill. Distance to fertiliser? I know some farmers would expect contractor to do 20 mile round trip back to farm for more bags.

£30 / hour. Minimum.

Anything else depends upon application rate, terrain, distance to yard, how wet and sticky the poorly stored sodding fert is.

£2 up to 2 cwt then plus £1 every cwt there after. I only do 1 neighbouring farm and its a min of 150 acres at a time every 2 weeks at 24m, 3 t capacity, auto steer, plus diesel charged from tractor fuel use monitor. Average 30 acres an hour.

Careful about the diesel charging. I'd expect some may get awkward about it not being calibrated weights/measures etc.
