Laying a small concrete slab for vehicle loading bay

I need to lay a slab for parking the chiller van (panel van) on such that it can be washed down and the run off collected in a drain.

How thick should the concrete be?
How thick should a stone sub-base be?
Do i shutter for both layers, so 8-10+ inches?
Best method of shuttering, given the above dimensions? Cheapest quality ply cut into lengths and strengthened by timber, with stakes into the ground to hold position? Or different?

Assume the easiest way of keeping liquid contained would be kerb stones or a brick upstand (i may have posed this question recently)?

You would get away with 4 inches of concrete if you are going onto a really good well compacted stone base, I'd make it all fall to the centre, or wherever the drain is
I don't think you need any kind of kerbstone
 
I was meaning the total height including subbase, does your comment still stand, 6" total?

Ah sorry. I thought you meant that was solid concrete.
As others have said, you'll get away with a relatively thin slab with a solid base. We always use mesh reinforcing but we're going over it with tractors so it might not be needed for lighter traffic.

Don’t know if you’ve got access to a decent vibrating roller but if you haven’t I’d rely on enough concrete strength so it moves/holds together as a slab rather than cracks.
