Once you see plants twisting and reacting to aphid attack is it too late to spray ? Are aphids likely to migrate to those plants unaffected? Is it worth it?

How do you get a spray on the underside of a horizontal leaf ? Makes black bean aphid spraying look easy.

What growth stage? Both Aphox and Biscaya have worked very well in peas for me this year.

probably 75-90% through flowering. infected plants look poorly. healthy plants filling in the gaps.

Difficult to say without seeing it obviously but I'd say if you've not finished flowering its definitely worth giving a go. PGRO guide is to treat when colonies can be found on 20% of plants. Many will probably be hiding in the upper leaves/flowers. Biscaya £8ish/acre from memory. Can't remember Aphox price nowadays but will likely be more predator friendly.
