We’ve has a vs 12 for a few years the only time I’ve had that happen was when the chains jumped the sprockets.
Turned out the shaft the drive sprockets sit on had sheared off in the middle.
Chains will also jump the sprockets when you run out of adjustment which puts a lot of strain on the floor slates.
