Accidents can happen without having to blame someone.Driver error in not doing pre-use checks properly?

be like some of the tourists that use the roads here, they pull them in so they don't get hit on the narrow roads. Arseholes.Optional mirrors?

But lorries still have accidents, ie lose wheels ,brakes failing , catching on fire. trailer un latching.Indeed, I was being partially facetious, but only partially - whilst I have no insight into this incident, annual maintenance, MOT checks and pre-use checks would pick up a lot of the mechanical failures that are manifest during operation.
Yes, the unforeseen can happen, but all of those you’ve mentioned can be prevented through appropriate maintenance and safety management. Isn’t it usually the case that events can be traced back to human error? Not always the driver, but maybe the mechanic who doesn’t notice a wearing part is at the end of its life, or the person who buys the wrong grade of bolts for the repair, etc. It’s really, really rare that an incident is genuinely random and not caused by somebody making a mistake. But lorries still have accidents, ie lose wheels ,brakes failing , catching on fire. trailer un latching.
Pushing to hard no maintenance or cleaning in between
