Try switching off the phone and starting it again in case its a software glitch. Also check the modes on the camera within the app in case its on macro or something. Point at something and touch the screen after ensuring its in the right mode and see if it auto focuses. Sorry if you have already tried these.

If nothing works, it might be worth a trip to a phone repairer to see what they can find.

If it is a very old model, like an iPhone 4, get it changed for a new 16 Pro Max. I have the current 15 Pro Max and the cameras are amazing and a giant leap forward from my previous iPhon8

Here are a couple taken today with the main lens of 15 Pro-Max running iOS18. Only one loads here because this is standard HEIF and the other is too big a file for the forum, being a HEIF MAX file

The rendering above does not do justice to how sharp and bright this and especially the higher definition photos look when viewed directly on my computers and screens. It is reduced here to under 600 KB which is pathetic, while the original Heif-Max is 7.7 MB and as good as most jpegs of 15 MB. Tried again and the high def is now reduced by this web site from 7.7 to 1.5MB. Still not in the same quality and clarity league as the original. This was exported to a file outside Photos app and attached as a file to here.
Not the camera’s fault...
