Depends on your vet how lucrative they are, I tell mine what I want on it and he fills it in.
What wormers/drenches/vaccinations used when and for what purpose.

What do you use or do if “bla bla” happens.... I think you can download the pre populated questions and then just fill in the answers and then a vet signs it. Been doing it for 20 years here as I’m organic.
 
Took about half an hour. Looked at my herd health etc plan as required by Assurance scheme and medicine record book then filled in a Health and Welfare review form. Basically he talked everything through, asked various questions and wrote down his recommendations. The aim is to reduce antibiotic use and reduce the chance of dose resistance. Questions on that were
1.Any evidence of recurring animal health problems
If yes list steps being taken to address them (ie you tell them any problems and how you deal with them)
2. Confirm that up to date medicine records have been reviewed including total antibiotic prescribed (he had a list of everything prescribed to check against the medicine book)
3 If antibiotics have been used suggest recommendations for responsible reduction without negative impact on welfare
4.If Hp CIAS used propose recommendations for reduction in future.
5.Prophylactic antibiotics on a significant scale are no longer acceptable practice. Please recommend alternative strategies
6 Is there a risk of developing anthelmintic resistance? Recommendations
7 Fluke problem ? Recommendations
8 Biosecurity and isolation Recommendations

With the industry pushing for less antibiotics and less worming, surely every health plan should simple say.

...."Worm lambs if needed. Use antibiotics if needed, use vaccines if needed"

and then sign and date it every year.

quite simple really
Just another one of those RT boxes to be ticked (filled in and an £100 to the vet)

Sit down with your vet for a half hours chat then receive a printed health plan that could apply to anyone and receive a bill for £100 job done ( or at least you have been ).

2018 quick discussion whilst doing another job. One page health plan that was basically a series of recommendations to buy treatments available from the Vet rather than cheaper alternatives. Cost £150

2019 Special visit by different Vet from normal. 50mins inc suckler herd. They have a list of when antibiotics were purchased or used by Vet on farm. Document with recommendations from Health review and antibiotic collation. Need to go on a course about medicines. Calendar type health plan.

There’s no reason you can't Do your own ‘calendar type’ health plan (for each flock & enterprise), if FA have decided it’s a requirement. I’ve not seen that requirement myself though. As long as your vet signs off your plan, it’s obviously been reviewed by your vet....

As part of an FC funded vet visit, I had my health plan updated and reviewed last week. No visit needed, but then he had been out that day doing some blood testing, and I have him on farm several times a year for various reasons. We sat at his computer in his office, where we discussed the ab’s we’d used over the last year, as well as any changes to my health plan since last year. It wouldn’t have taken an hour, even allowing for a cuppa and chatting around the houses. It will tick a box for the morons though.

On Thursday 26th September, we’re holding a webinar for farmers to go through the guidance, actions and detail for the expanded Sustainable Farming Incentive (SFI) offer. This was planned for end of May, but had to be delayed due to the general election. We apologise about that.

Farming and Countryside Programme Director, Janet Hughes will be joined by policy leads working on SFI, and colleagues from the Rural Payment Agency and Catchment Sensitive Farming.
