Knotter issue on small square baler

New holland bc 5060 baling today. Left hand knotter ties knot and cuts two tails but trails bale cord along on the bale leaving the left hand knotter unthreaded. It's does this 1 in 150 bales. The knotters aren't worn, right hand knotter works perfectly. There hasn't been an incident to cause this. Both knotters look identical. The only thing I noticed is when the left hand needle feeds cord up to the knotter there very occasionally appears to be a loop or twist in the the cord as the needle disappears to down its normal position again which i don't see happening on the right cord. This may be unrelated.

It only twisted a loop momentarily until the bale feeding into the chamber tensioned the twine again. It only does that on the one side. I put more tension on that tensioner on the twine box but I'll add a bit more in the morning to see if it helps further.

3rd on that list is twine tension.
We had grooves in the tensioners on our old big baler did similar. Had to take them apart and weld a bit in groove then grind back flat. I suppose could’ve bought new but we were too tight!

Ours is a newish baler, might have done about 75,000 bales but worth checking these things. I'll definitely put more tension on the twine. From that article I've a feeling there's a twist in the twine somewhere i dont think its a knotter problem as such. I'll have to go through everything on that article when I get the baler greased and blown down here

Found two problems. First I'd stupidly missed one of the eyes when threading.

2nd problem, the label appears to be a spool upside-down as there's a definite twist in one twine which the article says will cause the 3rd twine to twist and loop off the needle and go into the twine disk when knotting. I'll change this spool and tention the twine a bit more
